What Is Dental Fluorosis?

Fluorosis happens when a child swallows too much fluoride while their permanent teeth are still forming — usually under the gums, before age 8. Once the enamel is formed with this defect, no amount of brushing will remove it.

What causes fluorosis? Drinking water with fluoride above 1.5 mg/L during childhood tooth development. WHO and BIS both set this as the safe limit. Source: PubMed
Is fluorosis common in Haryana? Very common. A 2021 study across 2,200 children in endemic Haryana districts found 96.6% had dental fluorosis. Indian J Dent Res, 2021
Is fluorosis painful? Usually no. It mainly affects appearance. Severe cases can cause pitting and sensitivity, but pain is not a common symptom.
Can fluorosis be reversed? No. The enamel defect is permanent. However, the appearance can be corrected with veneers or crowns depending on severity.
Who gets fluorosis? Children who drank high-fluoride groundwater before age 8. Adults only show it — they cannot newly develop it. It is not contagious.
Does whitening work on fluorosis? Only for very mild white spots. It cannot fix brown staining or pitting. In moderate-to-severe cases, whitening can worsen the appearance.

Why Do Fluorosis Stains Not Go Away?

Think of your tooth enamel like a ceramic tile. The colour goes all the way through the material — not just the surface. Fluorosis gets locked into the enamel crystals when the tooth is forming. Once the tooth comes out of the gum, that defect is fixed permanently.

This is why whitening toothpaste and bleaching do nothing useful for moderate or severe fluorosis. The stain is not on the enamel — it is the enamel.

The only real solution is to place something over the front surface of the tooth. That is exactly what veneers do.

Questions About Fluorosis Treatment and Veneers

The best treatment depends on severity:

  • Mild fluorosis: Professional whitening or microabrasion may reduce white spots
  • Moderate fluorosis: Composite veneers or porcelain veneers give the most predictable result
