What is pulpotomy for kids?

Inside every tooth, even a small milk tooth, there is a soft center. We call it the pulp. It has nerves and blood vessels. That is why your child screams when something cold or sweet touches a bad tooth.

When a cavity goes deep enough, it reaches this pulp. It gets infected. It hurts. A pulpotomy is when we remove only the infected part of the pulp: the part inside the crown (the top of the tooth). The roots are left alone. The tooth is saved.

Think of it this way. If a mango has a small rotten spot, you cut out just that part and eat the rest. You do not throw the whole mango. That is exactly what a pulpotomy does for your child’s tooth.

How do I know my child needs a pulpotomy?

Your child will not always tell you clearly. Children often say “my tooth hurts a little” and then stop saying it. That does not mean the pain stopped, sometimes the nerve has died and the infection is getting worse quietly.

It is just a milk tooth. Why not pull it out?

This is the most common thing I hear from parents. And I understand why. Milk teeth fall out on their own anyway. So why spend money saving one?

Here is the thing. A milk tooth is not just temporary. It is holding space for the permanent tooth that will come underneath it. If you remove that milk tooth too early, the neighbouring teeth slide into the empty space. Now there is no room for the permanent tooth. It grows crooked, sideways, or gets stuck.

What early milk tooth loss actually does

Did You Know? Back molars (the grinding teeth) in children are not replaced by permanent teeth until age 11 to 12. If removed at age 5 or 6, the space stays empty for 6–7 years, long enough for serious bite problems to develop.

Will my child feel pain during pulpotomy?

This is every parent’s real worry. Not the cost, not the time. Just “will my child suffer?” The honest answer is no. A pulpotomy is done under local anesthesia, the same numbing injection used for fillings. Your child’s mouth is completely numb before we touch the tooth.

Soft foods only: khichdi, curd, mashed dal, soft roti. Avoid hard biscuits, chips, hard candy.

Day 2–3

Child can return to normal activities. Mild tenderness may still be present, this is normal and settles on its own.

What happens during a pulpotomy, step by step

The whole thing takes about 45 minutes to one hour. Most children do not even realise we are doing something serious because we keep talking to them through it.

X-ray and assessment

We take a quick X-ray to see how deep the infection is and check that the tooth roots are still healthy.

Gel + injection to numb

Numbing gel on the gum first, then a small injection of local anesthesia. The area becomes completely numb in 3–5 minutes.

Remove infected pulp

Only the infected portion (the crown pulp) is removed. The healthy root pulp stays. This is what makes it different from a full root canal.

Medicated dressing placed

A biocompatible material (MTA or ferric sulfate) is placed on the remaining pulp to calm it and stop bacterial growth.

Crown placed

A stainless steel crown (SSC) or a tooth-coloured zirconia crown is placed over the tooth to protect it and restore full chewing function.

If bleeding does not stop after the infected pulp is removed, it is a clear sign that the infection has gone deeper into the roots. In such cases, pulpotomy should not be done. Continuing will lead to failure. The correct treatment is then pulpectomy (full pulp removal) or, if the tooth cannot be saved, extraction. This is why an X-ray before treatment is not optional. It is mandatory at our clinics. Treating without a current X-ray is like operating without seeing inside.

Parents often confuse pulpotomy, pulpectomy (baby root canal), and full extraction. Here is a clear breakdown. The right treatment depends on how deep the infection has gone.

What about pulpectomy? Is it the same as root canal?

A pulpectomy is when the infection has reached the tooth roots, not just the crown. All the pulp is removed from both the crown and roots. Then the canals are filled with a special material that the body can dissolve as the permanent tooth grows in. In adults, we call this a root canal. In children, the material used is designed to resorb, so it does not block the permanent tooth. It is more complex than pulpotomy and costs a little more, but still saves the tooth.

Is it cheaper to just pull the tooth?
