Corrective Jaw Surgery (Orthognathic)
A specialized solution for severe skeletal misalignment where braces alone cannot fix the bite or facial profile.
When Do You Need Surgery?
Most bite issues are “dental” (crooked teeth) and can be fixed with Braces or Aligners.
However, if the size or position of your upper and lower jaws do not match significantly, it is a “skeletal” discrepancy. Orthognathic surgery repositions the jaw bones to correct:
- Severe Underbite (Protruding Chin)
- Severe Overbite (Receding Chin)
- Open Bite (Teeth don’t touch)
- Chronic Jaw Pain (TMJ)
- Obstructive Sleep Apnea
- Facial Asymmetry
- Difficulty Chewing or Biting
- Speech Problems
- Breathing Difficulties

Types of Corrective Surgery
Modern jaw surgery is performed entirely inside the mouth (no visible scars) using 3D Virtual Surgical Planning (VSP).
Maxillary Advancement (Le Fort I)
Corrects: Upper jaw deficiency, “Gummy Smile”, and Open Bite.
The surgeon separates the upper jaw (Le Fort I osteotomy) and moves it forward or upward to align with the lower teeth.
Mandibular Advancement (BSSO)
Corrects: Retruded lower jaw (weak chin) and severe overjet.
Using the BSSO technique, the lower jaw bone is split and slid forward to the new position, then secured with titanium plates.
Genioplasty (Chin Surgery)
Often combined with jaw surgery. The chin bone is cut and moved forward to dramatically improve the facial profile and neckline.
Bi-Maxillary Surgery
This involves operating on both the upper (MAX) and lower (MAS) jaws simultaneously. Essential for severe asymmetry or Sleep Apnea cases.
The Treatment Journey
Complete examination with X-rays, photographs, and 3D CBCT scan. We create a detailed surgical plan with virtual planning technology.
Braces are used to align teeth independently in each jaw, preparing them for the surgery. Regular check-ups every 4-6 weeks.
Performed under general anesthesia in a hospital. Duration is typically 2β4 hours. 1β2 days hospital stay. All cuts are made inside the mouth.
Initial swelling subsides in 2 weeks. Bones heal in 6 weeks. Soft diet is mandatory. Follow-up visits to monitor healing.
Fine-tuning the bite with elastics and braces to lock in the perfect occlusion. Retainers provided after braces removal.
What to Expect
- Surgery Day: You will be given general anesthesia and will be asleep during the entire procedure.
- Hospital Stay: Most patients stay 1-2 days in hospital for monitoring and initial recovery.
- Going Home: You can return home with pain medications and detailed care instructions.
- Follow-ups: Regular visits at 1 week, 2 weeks, 6 weeks, and 3 months to check healing progress.
Cost Perspective
Total cost includes Orthodontist Fees + Surgeon Fees + Hospitalization charges. The exact cost depends on complexity of your case.

Recovery & Diet After Surgery
What you eat during recovery is very important for proper healing. Your diet will progress through different phases.
Week 1-2: Liquid Diet Only
What you can have: Water, clear soups, protein shakes, smoothies, juice, coconut water, milk, lassi, butter milk, thin daal
Important: Do not use straw. Drink slowly from a cup or spoon. Stay hydrated by drinking at least 2-3 liters daily.
Tip: Add protein powder to milk or smoothies to maintain your strength and help healing.
Week 3-6: Pureed & Soft Foods
What you can have: Thick soups, mashed potatoes, khichdi, idli (very soft), curd, scrambled eggs, thick daal, banana, mashed vegetables, ice cream, yogurt
Important: Food should be soft enough that you can swallow without chewing. If it needs chewing, it is too hard.
Tip: Use a blender to make your regular food into smooth puree. Add ghee or butter for extra calories.
Week 7-12: Soft Chewable Foods
What you can have: Soft rice, well-cooked vegetables, soft roti, paneer, fish, minced chicken, soft fruits like papaya and mango
Important: Start chewing very slowly and gently. Stop if you feel pain. Avoid hard, crunchy, or chewy foods.
Avoid: Raw vegetables, hard fruits like apple, nuts, chips, papad, hard biscuits, popcorn
After 3 Months: Gradual Return to Normal Diet
You can slowly add harder foods back. Listen to your body. If jaw muscles feel tired, take a break. Your jaw strength will improve gradually.
Important: Continue to avoid very hard foods like sugarcane, hard candy, or extremely chewy items for at least 6 months.
Important Diet Tips
- Weigh yourself weekly. If you are losing weight, increase calories by adding ghee, butter, or protein supplements.
- Add fiber to prevent constipation. Drink prune juice or eat blended vegetables.
- Keep your mouth clean. Rinse gently with warm salt water after every meal.
- Prepare and freeze meals in advance so you have easy options ready.
Safety, Risks & Success Rate
High Success Rate
Studies show jaw surgery has over 95% success rate when performed by experienced surgeons. Serious complications are very rare.
Common Temporary Side Effects
These usually resolve within weeks to months:
- Swelling: Maximum at 3-5 days, goes down 90% by 8 weeks
- Numbness: Lips and chin may feel numb for 3-6 months as nerves heal. In 75% cases, sensation returns to normal
- Difficulty speaking: Improves as swelling reduces. Practice speaking to recover faster
- Jaw stiffness: Improves with gentle exercises after 4-6 weeks
- Bruising: Face may have bruising that fades in 2-3 weeks
Possible Risks (Rare)
These occur in less than 5% of cases:
- Infection: Treated with antibiotics
- Bleeding: Rarely requires additional intervention
- Permanent numbness: In about 1-2% of cases, some altered sensation may persist
- Bad split: In rare cases (1%), jaw may split differently than planned but can be repaired
- Jaw relapse: Jaw trying to move back to old position. Prevented with good planning and retainers
- TMJ problems: Very rare, but some patients may need additional treatment

Frequently Asked Questions
In mild to moderate cases, yes. We can use “Camouflage Orthodontics” (moving teeth to mask the skeletal issue). However, for severe discrepancies or sleep apnea, surgery is the only way to achieve a stable and healthy result.
The cost varies from βΉ60,000 to βΉ2,00,000 depending on the complexity. Simple single jaw surgery costs less. Double jaw surgery or cases with genioplasty cost more. The total includes orthodontist fees, surgeon fees, hospital charges, anesthesia, and follow-up care.
We provide detailed cost breakdown during your consultation so you know exactly what to expect.
Purely cosmetic jaw surgery is usually not covered. However, if the surgery is medically necessary (like for severe Sleep Apnea, TMJ dysfunction, or post-trauma/accident), some policies or corporate OPD benefits may offer partial reimbursement.
We can provide all medical documentation needed for insurance claims.
You will not feel any pain during surgery because of general anesthesia. After surgery, most patients say the discomfort is from swelling, not pain. We provide strong pain medications for the first week.
Most patients describe it as uncomfortable rather than painful. Pain reduces significantly after the first 3-4 days.
Initial recovery takes 2 weeks. You can return to school or office work after this. Swelling goes down 90% by 6-8 weeks. Complete bone healing takes 6 weeks. You should avoid contact sports or heavy exercise for 3 months.
Most patients feel back to normal routine within 4-6 weeks.
Most patients return to desk jobs within 2 weeks. If your work involves physical labor or talking a lot (like teaching), you may need 3-4 weeks off.
Plan for at least 2 weeks of rest at home. Working from home is possible after 1 week if needed.
No. All incisions are made inside your mouth. There will be no visible scars on your face or chin. This is one of the major advantages of modern orthognathic surgery.
Temporary numbness in the lip, chin, or cheeks is common because nerves near the jaw bones get stretched during surgery. In 75% of patients, normal sensation returns within 3-6 months as nerves heal.
About 20% may have slightly reduced sensation that improves over 12 months. Complete permanent numbness is very rare (less than 2%).
You may have difficulty speaking clearly for the first 2-3 weeks due to swelling and jaw stiffness. As swelling goes down, speech improves. Most patients return to normal speech within 4-6 weeks.
For some patients with severe jaw problems, jaw surgery actually improves their speech in the long term.
We usually wait until jaw growth is complete. For girls, this is around 16-17 years. For boys, it’s around 18-19 years. We take special X-rays to confirm growth has stopped before planning surgery.
In cases of severe sleep apnea or trauma, surgery may be done earlier under special circumstances.
No. Jaw surgery can be safely performed at any age as long as you are in good general health. Many patients undergo successful surgery in their 30s, 40s, or even 50s.
Older patients may take slightly longer to heal, but results are equally good.
Yes. In most cases, you need pre-surgical orthodontics (braces) for 9-18 months. This prepares your teeth for surgery by aligning them within each jaw.
Without braces, surgery results may not be stable, and your bite may not fit properly after surgery.
In very few cases where teeth are already well-aligned and only jaw bones need repositioning, surgery without braces might be possible. However, this is rare.
Most patients need both orthodontics and surgery for best results. We evaluate this during your consultation.
Yes, your facial appearance will change, but in a positive way. Jaw surgery improves facial balance and symmetry. Your profile will look more harmonious.
We use 3D planning software to show you predicted results before surgery so you know what to expect.
Yes. If your gummy smile is caused by excessive upper jaw (vertical maxillary excess), Le Fort I surgery can reposition your upper jaw upward, reducing gum display when you smile.
Yes. Jaw surgery (especially bi-maxillary advancement) is one of the most effective treatments for obstructive sleep apnea. It enlarges the airway by moving both jaws forward, allowing better breathing during sleep.
Studies show 85-90% improvement in sleep apnea symptoms after jaw surgery.
Many patients with TMJ pain caused by jaw misalignment find relief after surgery. When jaws are properly aligned, stress on the jaw joint reduces significantly.
However, if TMJ pain is from other causes (like arthritis), additional treatment may be needed.
Relapse (jaw moving back toward old position) is rare with modern surgical techniques and titanium plate fixation. Proper pre-surgical orthodontics and post-surgical braces also prevent relapse.
Following all post-operative instructions and wearing retainers is important to maintain results.
Before surgery, we take 3D CBCT scans, photographs, dental impressions, and lateral cephalogram X-rays. You will also need routine blood tests, ECG, and chest X-ray before hospital admission.
All these help us plan your surgery precisely and ensure you are fit for general anesthesia.
Single jaw surgery (upper or lower) takes 2-3 hours. Double jaw surgery takes 3-4 hours. If genioplasty is added, it may take an additional 30-45 minutes.
You will be under general anesthesia and will not feel anything during this time.
Yes. Most patients stay 1-2 days in hospital after surgery for monitoring and initial recovery. This ensures your airway is clear, swelling is managed, and pain is controlled before you go home.
For mild to moderate cases, orthodontics alone (braces or aligners) can camouflage skeletal issues by moving teeth. However, for severe skeletal discrepancies, surgery is the only way to truly correct the problem.
Non-surgical options cannot change jaw bone positions, only tooth positions.
Jaw surgery failure is very rare (less than 2-3% of cases). If results are not satisfactory or complications occur, revision surgery can be performed after proper healing.
With experienced surgeons and proper planning, the success rate is over 95%.
